Care Beyond Boundaries: The Woodlands Health Journey

Care Beyond Boundaries tells the story of Woodlands Health’s 10-year journey from greenfield to the opening of our Campus. Over the years, we’ve worked hard to shape a place that cares for residents in the North-Western region of Singapore, both inside and outside the hospital. We’ve set up Communities of Care in neighbourhoods, promoted healthy living and — in the process — brought people together. This book shares our experiences, the challenges we faced and the successes we achieved in our mission to provide Care Beyond Boundaries.

Woodlands Health is established, marking a new era in healthcare delivery. The new hospital is aimed at serving Singapore’s fast-growing North-Western region with a community-focused approach.

2016: For a healthier community

We began collaborating with community partners, and launched the SMART Community Nursing Team and Community Nurse Post two years later to provide comprehensive healthcare for residents.

18 April 2017: Breaking new ground

Our groundbreaking ceremony marked the start of construction and the realisation of our vision.

2017–2019: Pre-operations wards open

In October 2017, we opened our first pre-operations ward, D88, at Yishun Community Hospital. By December 2019, our network expanded to include additional wards at Yishun Community Hospital and Tan Tock Seng Hospital.

2020: Our COVID-19 response team in action

During the pandemic, Woodlands Health led medical efforts at Singapore’s first Community Isolation Facility at Pasir Ris D’Resort and the large-scale Community Care Facility at Singapore EXPO.

2020: Right-siting of care

In September 2020, we launched the GPFirst programme and Urgent Care Centre @ Admiralty to help residents receive the right care in the right place.

2021: Our Model of Care is launched

Based on the National Healthcare Group’s River of Life model, this framework guides our care delivery across the different stages — from pre-hospitalisation to integrated and continuing care — of our patients’ health journey.

2022: Community Health Posts open

We expanded our Communities of Care with these Community Health Posts (CHPs), which help residents take charge of their health, starting with the first CHP at Care Corner Active Ageing Centre (Marsiling) in December.

12 February 2022: Topping Out

The final structural beam was placed atop Tower B of our integrated acute and community hospital in a key event officiated by Minister for Health Mr Ong Ye Kung.

22 December 2023: Welcoming our first patients at our new Campus

We started with 40 community hospital beds, rehabilitative and sub-acute care services, and selected specialist clinic services. We also became the first hospital in Singapore to open with the integration of three national IT systems.

May 2024: The rest of our Campus opens

Our Emergency Department, Operating Theatres, acute hospital wards and Medical Centre were opened.

13 July 2024: Our official opening

The 7.66-hectare Woodlands Health Campus was officially opened by Senior Minister Mr Lee Hsien Loong.

2024 onwards: Our journey continues

With our comprehensive care services fully underway, we remain committed to enriching residents’ lives with good health.
